Spring Camp – Herndon

1. Parties to the Agreement

This agreement is executed between Kim’s Olympic Taekwondo Center (the "Center"), the undersigned Buyer (Parent/Guardian), and the Student(s) registered for the program.

2. Medical Information & Emergency Release

• Medical Conditions: I agree to provide accurate information regarding the Student's allergies, medications, or any pre-existing medical conditions.
• Emergency Authorization: I hereby authorize the Center to take necessary actions to provide emergency medical assistance for the Student(s).
• Financial Responsibility: I understand that I am solely responsible for any medical expenses, including hospital fees and transportation costs, incurred during such treatment.
• Emergency Contact: In case I cannot be reached, I will provide a secondary emergency contact person.

3. Liability Waiver and Release

• Risk Acknowledgment: I acknowledge that participating in martial arts involves strenuous physical exercise and inherent risks of injury. I voluntarily assume all such risks.
• Release of Liability: I hereby release and hold the Center, its instructors, and staff free from any and all liability, claims, or actions arising from injuries, accidents, or illnesses during camp.
• Property Damage: If a student or buyer damages the property of the Center or others, the Student and Parent/Guardian must indemnify and take full financial responsibility for repairs or replacement.

4. Photo and Media Consent

• I irrevocably authorize the Center to use, publish, and copyright photographs or videos of the Student(s) for promotional purposes (website, social media, brochures) without further compensation.

5. Fees and Late Pick-up Policy

• Returned Payments: A $20.00 fee will be assessed for any returned check or declined EFT.
• Late Pick-up: A $5.00 fee per student will be assessed for every 30 minutes past the 6:00 PM pick-up time.

6. Cancellation and Modification Policy

• No Refund Policy: Any payment received will not be refunded for any reason, including cancellation or modification of registration, except as required by law.

7. Student Pledge and Code of Conduct

The Student(s) agree to:

1. Prioritize the safety of self and classmates.
2. Use Taekwondo skills only for self-defense and honor.
3. Contribute to team unity and follow all staff instructions.
4. Disciplinary Action: Violations may lead to suspension, expulsion, or revocation of privileges without refund.

Acknowledgment and Digital Signature
